Easy cholangiography during laparoscopic cholecystectomy




Abstract
Laparoscopic cholecystectomy has become the standard tretment for symptomatic gall stone, but frequent accompanying hepaticocholedocholithiasis has been a major problem during laparoscopic biliary surgery especially in Korea. Known laparoscopic
cholangiographic catheter, which is fit to pass through 14G needle, was a little thick to pass through narrow cystic duct in some cases.
Therefore, we has tried to use a thin pediatric intravenous which is fit to pass through 18G needle, for insertion of it in 15 cases of narrow cystic duct at laparoscopic cholecystectomy successfully during last two years at the dept,. of
surgery,

@ES The results obtained were as follows.
@EN 1) The ratio of male to female was 1:1.5, and the most prevalent age was 5th decade, which accounts 47% of total. 2) The accompanying disease was 3 cases of diabetes mellitus 2 cases of hypertension, 1 case of pulmonary tuberculosis, and 1
case
of
bronchial asthma. 3) Location of hepaticocholedocholithiasis was 2(67%)cases of common bile duct, and 1(33%)case of intrahepatic duct. 4) Treatment of hepatiocholedocholithiasis was performed by laparoscopy of 1 case, mini-laparotomy of 1 case,
and
observation of 1 case.
A thin pediatric subclavian catheter, which is fit to pass through 18G needle, can be easily inserted through even in case of narrow cystic duct for laparoscopic cholangiography.
